To begin taking this survey, you’ll need to know your transaction number, store number, the date you made your purchase, and the register you were checked out at. You will be asked to rate your shopping experience, from the interactions you had with the staff, the cleanliness and appearance of the store itself, and if you found what you were looking for. When you answer these questions honestly, your answers are sent directly to the corporate offices and help shape how Ulta does business.
